David Blom, Special Assistant, Campus Title IX Coordinator | 608.890.3788 | | The UW-Madison Title IX Coordinator is designated to receive reports of sexual assault, sexual harassment, dating/domestic violence, stalking, and/or discrimination on the basis of sex.    
Dean of Students Office (DoSO) | 608.263.5700, TTY: 608-263-2400 | | DoSO is designated to receive reports of student misconduct.  Upon receiving a report, DoSO will ensure that victims are provided with assistance and referrals. DoSO can also provide information and assistance to students who are being investigated for allegations of misconduct.
Domestic Abuse Intervention Services (DAIS) | 608-251-1237, 24-Hour Help Lines: 608-251-4445 or 800-747-4045 | | An off-campus community agency providing free and confidential services for survivors of dating/domestic violence, stalking, including emergency shelter, legal advocacy, community education, and support groups.
UHS Counseling and Consultation Services | 608-265-5600 (option 2), 24-Hour Crisis Services: 608-265-5600 (option 9) | A confidential on-campus mental health resource for UW–Madison students, providing individual, couple/partner, and group counseling.
UHS End Violence on Campus (EVoC)   | 608-265-5600 (option 3) | | An on-campus confidential victim advocacy service for UW-Madison student victims of intimate partner violence, sexual harassment, stalking, and sexual assault.  Services include consultation, counseling & case management, accompaniment, support groups, and assistance in requesting accommodations from UW-Madison. UHS EVOC also provides violence prevention and training services.
